How to create RACI chart?



A RACI stands for Responsible, Accountable, Consulted and Informed. The method RACI Chart is usually served in Matrix (Matrix Chart) so often referred to with the RACI Chart or RACI Chart Matrix. In an organization, the RACI Chart is one of the tools that can be used for decision making and help the management in to identify the roles and responsibilities of its employees.

RACI chart is use as tools and techniques in “Develop Human Resource Management Plan” process. Which is part of Project Human Resource Management knowledge area and Initiating process group.

R.A.C.I = Responsible Accountable Consulted Informed

A clear division of tasks along with the roles and responsibilities it is important in an organization. These roles and responsibilities will result in confusion that will eventually result in a decreased work productivity of their employees.

The Four Parameters of The RACI Chart


RACI Chart is composed of four parameters, namely:

  • Responsible, i.e. the people who perform tasks or jobs. Responsible is essentially the incumbent.

  • Accountable, i.e. the responsible person on a task or a job and have the authority to decide a problem or matter. Accountable in question here is basically the person in charge and the decision makers.

  • Consulted, i.e. the person giving feedback, opinions or Contributions when necessary on the job or task. Consulted is basically a Consultant or advisor.

  • Informed, that people who need to know the actions and results or decisions that have been taken.

The steps of making the RACI Chart



  1. Identifies the process and desire job function in resolving the problems face. The problems of the process need to be investigate and studied thoroughly and set objectives or goals.

  2. Identify the role and functions of the work to be done, identify – the parties affect by these problems and who should do the job.

  3. Assign people who perform the task or job. Management needs to assign roles and responsibilities to each individual.

  4. Identify people who carry out the tasks/jobs (Responsible) and the parties entered into the list of A (accountable), C (consulted) and I (informed). RACI Diagram on this, there is only one person/party implement. While the A, C and I can consist of two or more parties.

  5. Revisiting these tasks so that duplication does not occur and the process of work.

In addition to the assignments of RACI, I have found it useful to extend RACI to add a Verify and Sign off properties - hence the term 'extend RACI chart'. In the past, I have found that we have need someone to verify the output of a task or that it has been complete fully. You could argue that the person accountable for the task should do this, but more than once I found that a task was report as complete and later it turn out that, although the task was technically complete, it was complete to the require quality.







































R: ResponsibleThe people who do the work to achieve the task. There is always at least one role of R, although this can also be delegated to others to carry out the work required, resulting in multiple R roles.
A: AccountableThe person who is ultimately accountable for the correct and thorough completion of the deliverable or task, and the one to whom R is accountable. In other words, an A must approves the work that R provides. There must be one and only one A specified for each task or deliverable.
C: ConsultedThese are people whose opinions are sought. This is a two-way communication interaction.
I:  InformedPeople who are kept up-to-date on progress, often only on completion of the task or deliverable. One-way communication.
V: VerifyThe person (or people) who check whether the product meets the acceptance criteria set forth in the product description.
S: Sign OffThose who approve the task completion.

Creating a RACI Matrix (step-by-step)



  • Identify all the tasks involve in delivering the project and list them on the left-hand side of the chart in completion order.

  • Identify all the project roles and list them along the top of the chart.

  • Complete the cells of the chart identifying who has responsibility, accountability and who will be consulted and inform for each task.

  • Ensure every task has a role responsible and a role accountable for it.

  • No tasks should have more than one role accountable. Resolve any conflicts where there is more than one for a particular task.

  • Share, discuss and agree the RACI Matrix with your stakeholders before your project starts.

The term RACI is an acronym for Responsible, Accountable, Consulted, and Informed. One or more of these properties are assign to each task that needs to be perform. During the various phases of a product development and release cycle.

That is, for each task, someone is responsible for performing that task, someone is accountable for the task being complete. One or more people might be consulted about the task, and one or more people might be inform about the task. Note that a given individual might be assign more than one property for the task i.e. They might be accountable and responsible. They are accountable more the task being complete and they do the work to complete the task.
